Four Arms is big and fat. He has four arms (duh) and a huge chin. He has a black stripe on his chin and forehead and has a mustache. He wears a black and green suit with the Omnitrix on his chest.

Young Four Arms looks the same except has no mustache, his suit is white and black, and he has the Omnitrix on his shoulder.

In EMS, Four Arms is shirtless and wears a green tie.
